Accessing Your Account
To access the Provider Portal, you must be invited by a current user. If you have have not received an account creation email, please contact your county administrator.
First Time Logging In

You should have received an email from admin@certifyos.com to access your account. This email contains login details, including a randomly generated password.

Navigate to https://provider.certifyos.com and enter your email and password and click “Login”.

While your current password is randomly generated and secure, it is strongly suggested that you change your password to a unique, secure password.
Select from Multiple Profiles
You might have multiple profiles to choose from if:

You have been invited to be an administrator for multiple organizations
-
You have been invited to be an administrator for an organization and as a Practitioner.
If this is the case, you will be prompted to select from a list of available profiles before proceeding.
Switching Profiles
If you have multiple profiles and would like to switch your current active profile:

Log out from the Portal.

Log in to the Portal.

Select the Profile you would like to access.
