Adding a Practitioner
The Add Practitioner functionality on the Practitioner List Page, allows an org admin to add new practitioners to the system through a pop-up form.
Accessing the Add Practitioner Functionality:

Log in to the portal as an org admin.

Navigate to the Practitioner List Page (the exact navigation path isn't visible in the screenshot).

Click the "Add Practitioner" button.

The "Add Provider" pop-up form will be displayed.
Add Practitioner Button:

Label: "Add Practitioner"

Functionality: When clicked, it opens the "Add Provider" pop-up form, allowing org admin to input information for a new practitioner.

Input Fields:

First Name*: Text field for the practitioner's first name (required field, indicated by the asterisk).

Middle Name: Text field for the practitioner's middle name.

Last Name*: Text field for the practitioner's last name (required field).

NPI*: Text field for the practitioner's National Provider Identifier (NPI) (required field).

Provider Type*: Dropdown menu to select the practitioner's type or designation (required field).

CAQH ID: Text field for the practitioner's Council for Affordable Quality Healthcare (CAQH) ID.

Operating States*: Dropdown menu to select the states where the practitioner is operating (required field).

Group*: Dropdown menu to select the group or organization the practitioner belongs to (required field).

External ID: Text field for an external identifier.

Last Credentialing Date: Date input field for the last credentialing date.

Next Credentialing Date: Date input field for the next credentialing date.

Provider Email Address*: Text field for the practitioner's email address (required field).

Provider Designation: Dropdown menu to select the provider's designation or role.

Employer: Text field for the practitioner's employer.

Close: Closes the "Add Provider" pop-up form without saving.

Add Provider: Saves the new practitioner information and adds them to the system.
