CAQH Client Maintenance and Attestation
What is included in the CAQH maintenance service?
Onboarding Steps
1. All Providers will need to have a CAQH Profile
2. Providers will need to provide login credentials in response to tasks related to enrollment
applications on the Platform.
3. Clinic administrator will need to provide Certify authorization to maintain their providers’ CAQH
Profiles.
4. Providers will need to provide Certify authorization to update and attest to his or her CAQH
profile.
5. Certify will pull the providers’ last CAQH profile attestation date and will begin monitoring future
attestation dates. Thirty days before the next attestation is due, Certify will notify the client and
provider and begin working on updating the CAQH profile.
6. All provider data in the Certify Platform will need to be the most up to date and active copy. If
Providers don’t have any updates to make prior to an upcoming attestation date, providers will
need to attest.
Ex: Malpractice COI or Professional Liability Insurance needs to be active (not expired) AND
providers or clinic administrator will need to upload the latest copy of the provider’s malpractice
insurance.
7. Once Certify has updated the provider’s CAQH profile, the provider will receive a notification via email to review a PDF copy of the updated profile in the Certify portal. Practice providers will need to review this PDF application prior to attestation to confirm accuracy and request any changes that may need to be made. Once accuracy is confirmed, the profile will be attested to.
